What is Bestatin (Ubenimex)?

Bestatin, also called ubenimex, is a small compound taken by mouth that supports the immune system by blocking enzymes that break down the peptide signals immune cells use to communicate 1. It is approved in Japan as an add-on cancer therapy, and in people with a form of acute leukemia it prolonged survival after remission. In the US it remains a research compound without FDA approval.

How it works in your body

Bestatin is a small molecule immunomodulator that enhances immune function by preventing the breakdown of important immune-signaling peptides. It works by inhibiting aminopeptidases (an enzyme that trims proteins), enzymes that degrade peptide hormones used by immune cells for communication. This extends the activity of these immune signals, boosting T cell function and natural killer cell activity against infections and cancer 1.

The deeper mechanism, for the curious

Bestatin (ubenimex) blocks three enzymes by competing with their normal targets: aminopeptidase (an enzyme that trims proteins) B (APB), leucine aminopeptidase (LAP), and leukotriene A4 hydrolase (LTA4H). 15 It boosts the immune system through several routes at once. (1) It blocks aminopeptidases (enzymes that trim proteins) on the surface of antigen-presenting cells (cells that show pieces of invaders to the immune system). This improves MHC class I antigen processing and presentation (the way cells display protein fragments to killer T-cells), so more CD8+ cytotoxic T-lymphocytes (killer immune cells) switch on. (2) Activated macrophages (immune cells that engulf invaders) make more interleukin-1 (IL-1). IL-1 drives T-helper cells (immune coordinator cells) to multiply and release IL-2, which strengthens the adaptive immune response (the part of immunity that learns and targets specific threats). 1 (3) It directly activates macrophages, so they engulf more, make more reactive oxygen species (chemicals that damage cells), and kill tumor cells better. (4) It stimulates bone marrow colony-forming units (CFU-GM, BFU-E, the starter cells for blood cells), which helps blood cell recovery after therapy that suppresses the bone marrow. 13 Its anti-inflammatory action centers on blocking LTA4H. This stops the conversion of leukotriene A4 into leukotriene B4 (LTB4), a strong attractant for neutrophils (a type of white blood cell) and a driver of inflammation. 5 In pulmonary arterial hypertension (high blood pressure in the lung arteries), LTB4 drives macrophage buildup, smooth muscle cell growth, and remodeling of the small lung arteries. 5 By lowering LTB4, bestatin may reduce these harmful processes. Its structure is a phenylalanine-leucine dipeptide analogue (a look-alike of a two-amino-acid unit) with a unique (2S,3R)-3-amino-2-hydroxy group. This lets it bind precisely to the zinc-containing active sites (the working pockets) of its target enzymes.

Immune

Bestatin is a well-studied immune booster that activates T-cells, macrophages, and natural killer cells (all immune cells) and increases IL-1 and IL-2 (signals that rev up immune cells). It is approved in Japan as an add-on immune treatment for acute non-lymphocytic leukemia (a fast-growing blood cancer), with decades of clinical evidence for its immune-boosting effects.

Healing & Recovery

By stimulating bone marrow stem cells to multiply and helping the immune system recover after chemotherapy, bestatin supports the body's rebuilding processes during cancer treatment. Clinical trials showed longer survival in AML patients (a type of blood cancer) who reach complete remission, meaning no detectable cancer.

Anti-Aging

Bestatin's ability to enhance immune surveillance and modulate inflammatory pathways through LTA4 hydrolase inhibition has potential implications for age-related immune decline and chronic inflammatory conditions, though direct anti-aging research is limited.

What the research points to

  • Blocks aminopeptidase (an enzyme that trims proteins) B and leucine aminopeptidase (enzymes that trim proteins), which helps the immune system spot threats
  • Pushes T-cells (white blood cells that attack infected or abnormal cells) to multiply and activate, improving cell-based immunity
  • Activates macrophages (immune cells that engulf invaders) and makes them better at swallowing tumor cells
  • Helps bone marrow stem cells multiply and recover after chemotherapy
  • Increases release of interleukin-1 (IL-1) and interleukin-2 (IL-2), two immune signaling proteins, from immune cells
  • Blocks leukotriene A4 hydrolase (LTA4H, an enzyme), which lowers production of leukotriene B4 (a chemical that drives inflammation)

The most-asked question

How much Bestatin (Ubenimex) do I take?

The research-reported range is 30 mg, once daily.

  1. Leukemia maintenance, Immune support

    30 mg

    • How often
      Once daily
    • For how long
      Long-term, alongside maintenance chemotherapy
    • How
      By mouth

30 mg by mouth once daily is the approved Japanese dose, used as add-on treatment after leukemia goes into remission (no detectable cancer). It helped patients, especially older adults, stay in remission longer 41.

Adjusting your dose

You're at the right dose if…

  • Stable or improving T-cell counts (infection-fighting white blood cells) on blood work
  • Staying in complete remission (no detectable cancer) in AML patients (a type of blood cancer)
  • Fewer infections during cancer treatment
  • Normal liver function tests during treatment

Consider going up if…

  • You are being treated for pulmonary arterial hypertension under physician guidance and require higher LTA4H inhibition
  • Your oncologist recommends dose escalation based on insufficient immune response markers
  • You have tolerated the starting dose well and are transitioning to the standard therapeutic dose

Go down if…

  • You experience persistent gastrointestinal side effects at the current dose
  • Liver function tests show elevated transaminases above acceptable thresholds
  • You develop significant skin reactions or allergic symptoms

The ceiling: 100 mg once daily (as established in Phase 1 clinical trials) 1

Reconstitution, in plain words

How do I mix Bestatin (Ubenimex)?

Peptides usually arrive as a freeze-dried powder in a small vial. “Reconstitution” just means adding sterile water back in so you can measure a dose. Here's how it works for Bestatin (Ubenimex):

What you need

  • Bestatin (Ubenimex) in its prescribed form
  • Clean, dry storage container
  • Measuring device if applicable (oral syringe, measuring cup)
  • Calendar or reminder app for dosing schedule

A worked example

Bestatin (Ubenimex) comes in pre-measured doses or forms. Follow the exact dosing instructions on your prescription label. No reconstitution or mixing is typically required for this formulation.

Working out your dose

Use Bestatin (Ubenimex) exactly as prescribed. Each unit contains the labeled amount. Your healthcare provider will determine the appropriate dose based on your individual needs and response.

Or let the calculator do it

Three quick questions — your vial, your water, your dose — and it tells you the exact syringe mark. “Find dose amount” is preloaded with Bestatin (Ubenimex)'s recorded schedule.

Timing

When do I take Bestatin (Ubenimex)?

Best time of day

Take Bestatin (Ubenimex) at the same time each day for consistent blood levels. Morning dosing with breakfast is often preferred, but follow your healthcare provider's specific instructions.

With or without food?

Bestatin (Ubenimex) can typically be taken with or without food. Taking it with a light meal may help reduce any GI discomfort. Avoid taking with grapefruit juice or high-fat meals unless specifically directed.

Taking it with other peptides

Bestatin (Ubenimex) should be used as directed by your healthcare provider. If combining with other medications or supplements, discuss potential interactions with your provider. Avoid combining with compounds that have overlapping mechanisms unless specifically guided by a medical professional.

The honest list

What side effects could Bestatin (Ubenimex) cause?

Common — usually mild

These show up most often. They usually fade as your body adjusts.

  • Stomach and gut discomfort

Mild nausea, diarrhea, or abdominal discomfort are the most frequently reported side effects with oral bestatin. These are typically transient and occur during the first week of treatment 10.

What to do Take with a small amount of food or water if stomach upset occurs. Symptoms usually resolve within a few days of continued use. If persistent, consider temporary dose reduction.

  • Skin rash or itching (pruritus)

Mild skin reactions including rash, itching, or urticaria have been reported in a subset of patients. These are generally mild and do not require discontinuation 10.

What to do Antihistamines may provide symptomatic relief. If rash is widespread or accompanied by systemic (affecting the whole body) symptoms, discontinue and consult healthcare provider. Usually resolves within days of stopping treatment.

  • Facial flushing

Transient facial flushing or warmth may occur, particularly during the initial days of treatment, likely related to immune activation and histamine release.

What to do Usually self-limiting and resolves within 30-60 minutes of dosing. No treatment required. If persistent or bothersome, dose reduction may help.

  • Fatigue

Mild fatigue or malaise may occur as the immune system is stimulated. This is more commonly noted in patients concurrently receiving chemotherapy.

What to do Ensure adequate rest and hydration. Fatigue typically improves after the first 1-2 weeks of treatment. May be managed with dose timing adjustment to evening if daytime fatigue is problematic.

Less common

Worth knowing about, but most people never see these.

  • Higher liver enzymes (blood tests that show liver stress)

Transient elevations in AST and ALT have been reported, particularly with higher doses or prolonged treatment. This is usually mild and asymptomatic 10.

What to do Regular monitoring of liver function tests is recommended, especially during the first months of treatment. If transaminases exceed 3x the upper limit of normal, consider dose reduction or discontinuation.

  • Mild drop in white blood cells (leukopenia)

A mild, usually transient decrease in white blood cell count may occur, particularly in patients already immunocompromised from chemotherapy.

What to do Monitor complete blood counts regularly. Mild leukopenia is often self-resolving. If white cell count drops significantly, hold treatment and consult oncologist.

Serious — stop and get medical help

Rare, but if any of these happen, stop and contact a medical professional.

  • Severe allergic reaction

In rare cases, severe allergic reactions can occur, including deep swelling of the face or throat (angioedema) or symptoms resembling a whole-body allergic emergency (anaphylaxis). This is extremely uncommon given how small the bestatin molecule is.

What to do Discontinue immediately and seek emergency medical attention. Patients with a history of severe drug allergies should be monitored closely during initial dosing. Have emergency medications available for first doses.

Week by week

What should I expect from Bestatin (Ubenimex), and when?

  1. Week 1-2

    You might notice
    • The immune system starts to ramp up as bestatin blocks aminopeptidase (an enzyme that trims proteins)
    • IL-1 and IL-2 (signals that rev up immune cells) increase within days of starting treatment
    • A mild boost in energy as immune function begins to improve
    What's normal
    • Mild stomach discomfort or facial flushing during the first few days
    • No dramatic outward changes — the immune effects start inside your cells before you can feel anything
    • Blood work may not yet show measurable changes in your immune numbers
    What's next
    • Continue consistent daily dosing at the prescribed amount
    • Immune activation cascades are building but take time to become clinically apparent
    • First blood tests may be scheduled to monitor baseline immune cell counts
  2. Week 2-6

    You might notice
    • T-cell (infection-fighting white blood cell) counts and activity rise on blood work
    • Macrophages (immune cells that engulf germs) work better, and general immune markers improve
    • In cancer patients: better tolerance of ongoing treatment
    • General improvement in well-being and fewer infections
    What's normal
    • Immune markers improve gradually rather than suddenly
    • Some ups and downs in blood counts are normal while the immune system recovers
    • In AML patients (a type of blood cancer): staying in complete remission, meaning no detectable cancer, is the main goal
    What's next
    • Continue treatment as prescribed — benefits accumulate with sustained use
    • Regular blood monitoring to track immune recovery and liver function
    • For AML patients: combination with standard maintenance chemotherapy continues
  3. Week 6-24+

    You might notice
    • Sustained immune enhancement with stable T-cell and NK cell populations
    • In AML patients: prolonged disease-free survival during maintenance phase
    • Reduced frequency of opportunistic infections during cancer treatment
    • For PAH patients (investigational): potential improvement in exercise capacity and hemodynamics
    What's normal
    • Immune benefits plateau at a maintained enhanced level
    • Long-term treatment is well-tolerated in clinical studies with minimal cumulative toxicity
    • Periodic blood monitoring remains important for long-term therapy
    What's next
    • Continue long-term maintenance as directed by oncologist or prescribing physician
    • Treatment duration in AML is typically 1-2 years or as part of ongoing maintenance protocol
    • Discuss with physician whether dose adjustment is needed based on immune monitoring

How to tell it's actually working

Immune system test results

  • More T-cells (infection-fighting white blood cells), including CD3+ and CD4+ types, on blood work
  • A better CD4/CD8 ratio (the balance between two kinds of T-cells), showing a balanced immune system
  • More active natural killer cells (immune cells that attack tumors and infected cells) on lab function tests
  • Stable or improving white blood cell counts during chemotherapy

Health outcomes seen by doctors

  • Staying in complete remission (no detectable cancer) in AML patients (a type of blood cancer)
  • Fewer infections during cancer treatment
  • Better energy and general well-being
  • No signs of the disease returning on regular monitoring

Not seeing anything? The usual reasons

  • Not taking it consistently. Bestatin needs daily dosing to keep blocking aminopeptidase (an enzyme that trims proteins)
  • Dose too low. If you use 10-20 mg and see no immune benefits, talk to your physician about moving up to 30 mg
  • Immunosuppressive treatment at the same time, which works against bestatin's immune-boosting effects
  • Advanced disease, where boosting the immune system alone is not enough without adequate chemotherapy
  • Natural differences between people in aminopeptidase levels may change how well it works

Is it right for you?

Who should avoid Bestatin (Ubenimex)?

Do not use it if…

  • You have a known allergy or hypersensitivity to bestatin or similar drugs that block aminopeptidase (an enzyme that trims proteins)
  • You are pregnant or breastfeeding. Safety in pregnancy has not been established
  • You have severe, uncompensated liver disease. Bestatin may make liver function worse
  • You have an autoimmune disease that is flaring, where boosting the immune system could be harmful

Be extra careful if…

  • You are taking immunosuppressive medications. Bestatin's immune-boosting effects may work against them
  • You are taking ACE inhibitors (blood pressure drugs). Bestatin may add to their blood-pressure-lowering effect because both act on related aminopeptidase (an enzyme that trims proteins) pathways
  • You have moderate liver impairment. Monitor liver function closely during treatment
  • You have a history of severe drug allergies or atopic conditions (such as eczema or asthma)

When to stop

  • Severe allergic reaction (rash, swelling, difficulty breathing)
  • Significant liver enzyme elevation (AST/ALT >3x upper limit of normal)
  • Persistent or worsening side effects despite dose adjustment
  • Disease relapse requiring change in treatment protocol
  • Physician determines treatment goals have been achieved or treatment is no longer beneficial

Always consult with your healthcare provider before stopping bestatin, especially during cancer treatment. Abrupt discontinuation during AML maintenance therapy should only be done under medical supervision.

What it mixes badly (and well) with

Medications

  • OK together
    Chemotherapy agents (cytarabine, daunorubicin)

Bestatin is specifically designed and approved to be used alongside AML (acute myeloid leukemia, a fast-growing blood cancer) chemotherapy. Clinical trials showed combined benefits with standard induction and maintenance regimens (the first and the follow-up phases of chemotherapy).

  • Avoid
    Immunosuppressants (cyclosporine, tacrolimus)

Bestatin boosts the immune system, which directly works against drugs that hold it back. Avoid using them together, or use them together only with careful immune monitoring.

  • Avoid
    ACE inhibitors

Bestatin blocks aminopeptidases (enzymes that trim proteins) involved in handling angiotensin (a blood pressure hormone), which may make the blood-pressure-lowering effect of ACE inhibitors stronger. Monitor blood pressure closely if used together.

  • OK together
    Antibiotics

No known negative interactions with common antibiotics. Bestatin may support antibiotic treatment by helping the immune system clear infections.

Other peptides

  • Check first
    Thymosin Alpha-1

Both boost immune function in ways that complement each other. Thymosin Alpha-1 helps T-cells (immune cells that attack infected or abnormal cells) mature in the thymus (a gland in the chest), while bestatin helps T-cells in the rest of the body switch on. The combination may give extra immune support but should be watched for too much immune stimulation.

  • OK together
    Thymopentin (TP-5)

Boosts the immune system in ways that complement bestatin. TP-5 adjusts how the thymus (a gland in the chest) works, while bestatin acts on aminopeptidases (enzymes that trim proteins). No known negative interactions, but the combination has not been studied much.

  • OK together
    BPC-157

BPC-157 focuses on tissue repair while bestatin targets immune function. Their effects do not overlap and no interactions are known. Can likely be used at the same time without problems.

Supplements

  • OK together
    Vitamin D

Vitamin D supports immune function in ways that complement bestatin and may strengthen bestatin's immune-boosting effects. Safe to combine.

  • OK together
    Zinc

Zinc is essential for immune cells to work properly and for T-lymphocytes (immune cells that attack infected or abnormal cells) to develop. May support bestatin's immune-boosting effects. Safe to combine.

  • OK together
    Green tea extract (EGCG)

EGCG (a compound in green tea) can also block aminopeptidases (enzymes that trim proteins), which may overlap with bestatin. While generally safe, combining several aminopeptidase blockers may add up their effects. Use with awareness.

Who it tends to suit

Added immune therapy for AML (acute myeloid leukemia, a fast-growing blood cancer)

Bestatin is approved in Japan for adults with acute non-lymphocytic leukemia (a fast-growing blood cancer) who are in complete remission, meaning no detectable cancer, after their first round of chemotherapy. 10 Clinical trials showed that adding bestatin to standard maintenance treatment significantly lengthened the time patients stayed free of disease 34.

Post-Chemotherapy Immune Recovery

Bestatin helps the bone marrow make more stem cells (the cells that become blood cells) and helps T-cells and macrophages (immune cells) work better. This supports immune recovery in patients whose immune system was weakened by chemotherapy 13.

High Blood Pressure in the Lung Arteries (Pulmonary Arterial Hypertension, still being tested)

Through inhibition of LTA4 hydrolase and reduction of pro-inflammatory leukotriene B4, 5 bestatin is being investigated as a novel approach to PAH. Eiger BioPharmaceuticals conducted Phase 2 trials (NCT02664558) 7 and received FDA orphan drug designation for this indication 9.

Managing Lymphedema (swelling from fluid buildup, still being tested)

Bestatin calms inflammation by blocking the LTB4 pathway (a chemical route that drives inflammation). That led to a Phase 2 trial (NCT02700529) 8 in lymphedema (chronic swelling from fluid build-up), a condition with few treatments where inflammation plays a key role.

The safety picture overall

Bestatin (ubenimex) has an established safety record from decades of clinical use in Japan at 30 mg daily to help keep AML (a type of blood cancer) in remission. 310 It is generally well tolerated; the most common side effects are mild stomach upset and skin reactions. 10 Japan's long-term monitoring after approval has not found major safety concerns at standard doses. Phase 2 trials in the US for pulmonary arterial hypertension (high blood pressure in the lungs' arteries) and lymphedema (chronic limb swelling) 78 also showed acceptable safety at higher doses.

Safety data is supported by Japanese regulatory approval, Phase 1-2 clinical trials, and decades of post-marketing surveillance. FDA orphan drug designation for PAH 9 indicates adequate safety data for continued clinical development. Most safety data comes from Japanese clinical experience; data in non-Japanese populations is more limited.

Keeping it good

How do I store Bestatin (Ubenimex)?

Before mixing (powder)

Store Bestatin (Ubenimex) in the refrigerator at 36-46°F (2-8°C) in its original packaging. Protect from light and moisture. Do not freeze. Check the expiration date before use. Some formulations may be stored at room temperature for limited periods—check your specific product labeling.

After mixing (liquid)

Once reconstituted, Bestatin (Ubenimex) should be kept refrigerated at 36-46°F (2-8°C) and used within the timeframe specified on your product labeling (typically 14-28 days). Label the vial with the reconstitution date. Do not use if the solution appears cloudy, discolored, or contains particles.

Signs it has gone bad

  • Solution appears cloudy, discolored, or contains visible particles (should be clear)
  • Product has been exposed to temperatures outside the recommended storage range
  • Product has been frozen (unless specifically designed for freeze-thaw stability)
  • Expiration date has passed or reconstituted solution has exceeded its use-by date
  • Unusual odor, color change, or visible contamination

What the studies actually found about Bestatin (Ubenimex)

1987

[A new antitumor drug with immunomodulating activity, ubenimex (bestatin)]

Bestatin works by blocking enzymes on immune cells, which lets important immune signals last longer and boosts T-cell activity (T-cells are immune cells that attack infected or abnormal cells). When mice with cancer got bestatin combined with chemotherapy, they lived much longer than those getting chemo alone. This shows the drug strongly strengthens the body's cancer-fighting immune response.

Tsukagoshi S

1991

Review of ubenimex (Bestatin): clinical research

AML patients (people with acute myeloid leukemia, a fast-growing blood cancer) who took bestatin 30 mg daily after chemotherapy put them in remission stayed cancer-free much longer than those on placebo. The peptide worked by cranking up production of IL-1 and IL-2, the chemical messengers that rev up immune cells to attack any remaining cancer cells.

Blocking macrophage leukotriene b4 prevents endothelial injury and reverses pulmonary hypertension

Scientists discovered bestatin blocks an enzyme that makes leukotriene B4, a strong inflammation-causing chemical that damages lung blood vessels. This anti-inflammatory action opened up a completely new use for bestatin: possibly treating pulmonary arterial hypertension (high blood pressure in the lung arteries), a serious lung disease where inflammation scars the arteries.

Bestatin (Ubenimex) FAQ

What is the difference between bestatin and ubenimex?

Bestatin and ubenimex are two names for the same compound. "Bestatin" is the original laboratory name given by its discoverer Hamao Umezawa in 1976, 6 while "Ubenimex" is the official international drug name used for the approved pharmaceutical product. In Japan, it is sold under the brand name Bestatin by Nippon Kayaku Co., Ltd. 910 Both names refer to the same small two-part protein fragment that blocks aminopeptidases (enzymes that trim proteins).

Is bestatin approved by the FDA?

No, bestatin (ubenimex) is not currently FDA-approved. Japan has approved and used it since the 1980s as an add-on immune therapy for acute non-lymphocytic leukemia (a type of blood cancer). 10 In the United States, it has FDA orphan drug status (a designation for rare-disease treatments) for pulmonary arterial hypertension (high blood pressure in the lungs' arteries), 9 and Phase 2 clinical trials have been run for that condition and for lymphedema (chronic limb swelling) by Eiger BioPharmaceuticals. 78 While not FDA-approved, decades of Japanese clinical use back up its safety and effectiveness.

How does bestatin differ from other immune supplements?

Bestatin is a pharmaceutical-grade compound with a very specific job—it blocks three enzymes: aminopeptidase (an enzyme that trims proteins) B, leucine aminopeptidase, and LTA4 hydrolase. 15 General immune supplements broadly rev up immune activity. Bestatin instead works through precise enzyme-blocking that helps immune cells recognize threats, activates T cells (a kind of white blood cell), and improves the work of macrophages (immune cells that swallow germs)—while also cutting production of leukotrienes (chemicals that drive inflammation). It has been tested in randomized clinical trials and holds regulatory approval in Japan, 10 setting it apart from over-the-counter immune products.

Can bestatin be used for conditions other than leukemia?

Yes. Bestatin was developed and approved for leukemia, but the way it works has opened other doors. Eiger BioPharmaceuticals ran Phase 2 trials for pulmonary arterial hypertension (high blood pressure in the lungs' arteries, NCT02664558) 7 and lymphedema (chronic limb swelling, NCT02700529), 8 using bestatin's ability to block LTA4 hydrolase (an enzyme that drives inflammation and harmful blood-vessel changes). 5 Research has also explored its potential in solid tumors, autoimmune conditions, and as a broad immune booster for people with weakened immune systems.

What is the recommended dose of bestatin?

The standard therapeutic dose used in Japanese clinical practice is 30 mg once daily, taken orally. 10 Phase 1 clinical trials established an optimal dose range of 10-100 mg daily. 1 For leukemia maintenance, 30 mg daily is standard. 3 For investigational indications like PAH, higher doses may be used under physician supervision. Treatment duration varies by indication — in AML maintenance, courses typically extend for 1-2 years or longer.

How was bestatin discovered?

Bestatin was discovered in 1976 by Professor Hamao Umezawa and his team at the Institute of Microbial Chemistry in Tokyo, Japan. They isolated it from a soil bacterium called Streptomyces olivoreticuli. 16 Umezawa was a pioneer in finding enzyme-blocking compounds from microbes and discovered other important compounds too. Bestatin turned up in a screen for small molecules that block aminopeptidases (enzymes that trim proteins), and its immune-boosting and anti-tumor properties were worked out afterward.

Are there serious side effects with bestatin?

Bestatin is generally well-tolerated, with most side effects being mild and transient. Common effects include mild GI discomfort, skin rash, 10 and facial flushing. Uncommon effects include transient liver enzyme elevations and mild decreases in white blood cell counts. Serious side effects such as severe allergic reactions are rare. Long-term Japanese clinical experience over decades has confirmed a favorable safety profile at the standard 30 mg daily dose. 34 Liver function monitoring is recommended during treatment.
