What is ARA-290 (Cibinetide)?

ARA-290, also called cibinetide, is a lab-made peptide derived from erythropoietin, the hormone best known for boosting red blood cells. It was designed to keep erythropoietin's tissue-protecting, anti-inflammatory side without raising red blood cell production. It is being studied mainly for nerve damage and nerve pain, such as in diabetes, and has reached Phase 1-2 human trials; it is not FDA approved.

How it works in your body

ARA-290 mimics a naturally occurring peptide (erythropoietin fragment) that reduces inflammation and protects nerves from damage. It's being developed for conditions like diabetes-related nerve pain where inflammation damages nerve tissues.

The deeper mechanism, for the curious

ARA-290 (cibinetide) is a peptide derived from erythropoietin (EPO, the hormone that makes red blood cells). It switches on a two-part receptor (CD131/βcR) without triggering erythropoiesis (red blood cell production). It sends anti-inflammatory and tissue-protecting signals through JAK/STAT pathways (an internal messaging route in cells). This lowers production of inflammatory cytokines (signaling proteins) and improves nerve cell survival in diabetic neuropathy (nerve damage from diabetes).

Evidence check

Some human studies

It has been tested in people, but only in small or early-stage studies. Promising, but not yet proven the way an approved medicine is.

Every numbered marker on this page links to one of the 6 sources at the bottom.

95/100

Nerve Regeneration

ARA-290 actively regrows small nerve fibers that die from diabetes, neuropathy, or injury. Studies show real improvements in nerve density on corneal imaging.

85/100

Pain Management

Reduces neuropathic pain through both nerve repair and anti-inflammatory pathways. Users report decreased shooting pain and burning sensations.

90/100

Tissue Protection

Protects pancreatic cells, organs, and tissues from stress damage. Works especially well in diabetes where tissues face constant oxidative harm.

What the research points to

  • Regrows small nerve fibers damaged by neuropathy (nerve damage)
  • Reduces chronic nerve pain and discomfort
  • Decreases inflammation throughout the body
  • Protects tissues from damage and oxidative stress (damage caused by unstable oxygen molecules)
  • Improves blood sugar (glucose) control in type 2 diabetes
  • May protect pancreatic cells (cells that make insulin) in transplantation

Adjusting your dose

You're at the right dose if…

  • Therapeutic goals being met with minimal side effects
  • Stable and consistent response to treatment
  • Lab values or clinical markers trending in the right direction
  • Good tolerance with manageable or absent side effects

Consider going up if…

  • You've tolerated the current dose for the recommended period without significant side effects
  • Therapeutic goals haven't been met at the current dose level
  • Your healthcare provider recommends dose escalation based on your response
  • Lab work or clinical assessments support a higher dose

Go down if…

  • Side effects are bothersome or impacting daily life despite management strategies
  • You experience any signs of an adverse reaction
  • Lab results indicate the need for dose reduction
  • Your healthcare provider recommends a lower dose based on your response

The ceiling: 0.8 mg (as determined by your healthcare provider based on individual response and tolerance)

Reconstitution, in plain words

How do I mix ARA-290 (Cibinetide)?

Peptides usually arrive as a freeze-dried powder in a small vial. “Reconstitution” just means adding sterile water back in so you can measure a dose. Here's how it works for ARA-290 (Cibinetide):

What you need

  • ARA-290 (Cibinetide) vial (lyophilized powder or solution)
  • Bacteriostatic water or sterile sodium chloride for reconstitution
  • Alcohol swabs for cleaning vial tops and injection sites
  • Appropriately sized syringes with fine-gauge needles (27-30 gauge)
  • Sharps disposal container

A worked example

Add the recommended volume of bacteriostatic water to the ARA-290 (Cibinetide) vial. Gently swirl (do not shake) until the powder is fully dissolved. The resulting solution should be clear. Calculate your individual dose based on the concentration and your prescribed amount.

Working out your dose

Your dose of ARA-290 (Cibinetide) is determined by your healthcare provider. Using an insulin syringe marked in units, draw up the exact amount prescribed. For example, if the reconstituted concentration is 1mg/mL and your dose is 0.5mg, draw up 0.5mL (50 units on an insulin syringe). Always double-check calculations before injection.

Or let the calculator do it

Three quick questions — your vial, your water, your dose — and it tells you the exact syringe mark. “Find dose amount” is preloaded with ARA-290 (Cibinetide)'s recorded schedule.

Injection sites & technique

Where do I inject ARA-290 (Cibinetide)?

The best spots

  • Abdomen (stomach area)—at least 2 inches from the belly button, most popular choice for self-injection
  • Front of thighs—middle to upper portion of the outer leg
  • Back of upper arm—outer area (may need assistance from another person)

How to do it, step by step

  • Wash your hands thoroughly with soap and water before handling supplies
  • Clean the injection site with an alcohol swab and let it air dry completely
  • Pinch a fold of skin at the chosen injection site
  • Insert the needle at a 45-90 degree angle (depending on needle length and body composition)
  • Inject the medication slowly and steadily over 5-10 seconds
  • Release the skin fold and remove the needle, applying gentle pressure with a clean swab
  • Rotate injection sites to prevent tissue irritation or lumpy fat changes under the skin (lipodystrophy (lumpy or dented fat under the skin at injection sites))
  • Dispose of the needle safely in a sharps container—never recap or reuse needles

Subcutaneous (under the skin) injection (into the fatty tissue just under the skin)—allows for consistent absorption and can be self-administered at home after proper training

The safety picture overall

ARA-290 is a lab-made peptide that switches on the body's built-in repair system (the innate repair receptor). It has completed Phase 2 clinical trials for small fiber neuropathy (damage to the small nerves that sense pain and temperature) and is under active development. Animal toxicity studies show a good safety margin, with no organ damage or harm to cells at doses far above treatment levels. Early Phase 2 data show it is well tolerated, with mild injection-site reactions and passing headaches the main concerns. However, with repeated dosing of lab-made peptides there is a risk the immune system starts attacking the drug, so monitoring for antibodies against it is required. Long-term safety data beyond 12 weeks does not yet exist.

Evidence comes from preclinical (animal or lab studies, not human trials) toxicology studies, Phase 1 dose-escalation and safety data, and Phase 2 efficacy trials in small fiber neuropathy patients. The phase 2 trial enrolled 127 patients with a primary endpoint of safety and tolerability. No serious adverse events directly attributable to ARA-290 were reported in early studies. Data suggests good safety at doses up to 1 mg subcutaneously (under the skin) twice weekly, but longer-term outcomes and immunogenicity assessments continue.

Keeping it good

What the studies actually found about ARA-290 (Cibinetide)

2015

ARA 290, a nonerythropoietic peptide engineered from erythropoietin, improves metabolic control and neuropathic symptoms in patients with type 2 diabetes

ARA-290 treatment clearly reduced nerve pain in people with diabetes and increased corneal nerve fibers (nerve fibers on the surface of the eye). The peptide showed disease-modifying properties (it changed the course of the disease, not just the symptoms), with lasting benefits for both pain-related and non-pain-related symptoms.

Brines M, et al.

2014

Erythropoietin-derived nonerythropoietic peptide ameliorates experimental autoimmune neuritis by inflammation suppression and tissue protection

ARA 290 greatly improved recovery from nerve inflammation caused by the immune system attacking the nerves. It did this by helping nerves regrow and rebuild their protective coating (remyelination) while calming inflammation. The treatment increased protective regulatory T cells (immune cells that keep the immune system in check) and reduced inflammatory immune responses.

Liu Y, et al.

2014

ARA 290 for treatment of small fiber neuropathy in sarcoidosis

In people with sarcoidosis (an inflammatory disease), ARA 290 treatment was linked to clear improvements in nerve pain, more corneal nerve fibers (nerve fibers on the surface of the eye), better sensory thresholds (the ability to feel light touch and temperature), and better quality of life. Its excellent safety profile and its disease-modifying properties (it changes the course of the disease, not just the symptoms) make it promising for treating this orphan disease (a rare disease).

Your questions

ARA-290 (Cibinetide) FAQ

How is ARA-290 different from regular erythropoietin (EPO, the hormone that makes red blood cells)?

ARA-290 is an 11-amino acid fragment derived from EPO. While EPO stimulates red blood cell production, ARA-290 keeps only the tissue-protective, anti-inflammatory, and nerve-regenerating effects. It activates different cellular pathways without causing the problematic increase in red blood cells that EPO does.

How long before I see results from ARA-290?

Most people notice initial improvements like better sleep or reduced inflammation within 1-2 weeks. Significant pain reduction typically appears by week 6-8. However, nerve fiber regeneration (visible on corneal scans) takes 8-16 weeks to become really apparent. Patience is key with this one!

Can ARA-290 cure neuropathy permanently?

ARA-290 regenerates nerve fibers and reduces pain, which is remarkable. However, it's best thought of as a treatment that needs to be continued, especially if the underlying cause (like diabetes) isn't completely controlled. Some improvements may be permanent, but ongoing treatment typically provides best results.

Is ARA-290 safe for diabetics?

Yes, and it's actually particularly helpful for diabetics with neuropathy. Clinical trials showed it improves glucose control and reduces neuropathic pain in type 2 diabetics. However, blood pressure and red blood cell counts should be monitored, as with any peptide therapy.

Can I use ARA-290 if I have cancer history?

This is one of the contraindications. Because ARA-290 activates cell growth and repair pathways, it's considered unsafe for people with active cancer or recent cancer history. Always discuss your complete medical history with your doctor before starting.

How do I inject ARA-290?

Subcutaneous (under the skin) injection (under the skin) is preferred. Common sites include the abdomen, thigh, or upper arm. Some clinical trials used intravenous administration, but subcutaneous is easier for self-administration. Your doctor will provide detailed injection training.

What happens if I stop taking ARA-290?

Many benefits persist because nerve fibers that regenerate tend to stay regenerated. However, inflammation may gradually return and pain might increase over time. This is why continued treatment or maintenance dosing is usually recommended for best long-term results.
